Fees for Services:

Initial Mental Health Assessment 90-120 minutes:  $250

Initial Couple Assessment 90-120 minutes: $300

Consultation fee: $200

Psychotherapy sessions of 50-60 minutes:  $150

Psychotherapy sessions of 38-45 minutes:  $125

Psychotherapy with couples: First 60 minutes: $200. Each additional 30 minutes $100

Psychological testing, interpretation, and report preparation by professional psychologist:  60 minutes:  $210

Discussion of assessment results (in person or by phone): $200

(Completion of a separate contract is required to request psychological testing services.)

Missed appointment or cancellation in less than 24-hours: $75

Reduce fee services are available on a limited basis. Please consult with me if you need help in covering the cost of psychological services.

 

Payments

Payment in full or co-payments if using health insurance will be collected at the start of each session. Cash, checks or credit cards are acceptable methods of payment. I will provide you with a receipt for all fees paid. There will be a $35 service charge for all returned checks.  Non-payment of fees can result in termination of professional services and collection activity for any outstanding balance. 

Health Insurances:

If you have health insurance, I will work with you and your insurance provider to determine benefits, such as your copayment and number of sessions per year covered, and obtaining authorization for services if needed. It is your responsibility to contact your health insurance to collect this information prior to our first session. You must bring a current insurance card for the first appointment. Please report any changes related to your insurance plan including if a new card is issued to you as soon as possible. In the event that your health insurance does not pay for services provided, I will bill you directly for past and ongoing sessions at the regular fee established for sessions. If you fail to pay or you are unable to pay, termination of services will be considered and you may be referred to another provider.

Other Fees

During the course of treatment, other services may be needed including administrative support, consultations, and legal proceedings. Unfortunately, insurance companies do not reimburse for these services because there is not related to a medical necessity. Therefore you will be responsible for these fees.

 Administrative support fees: $ 100.00 per hour.

Administrative support fees refer to the writing of any supportive documentation per your request and/or writing letters to other professionals or reports in reference to your treatment ( Example: letters to primary providers, for employment, for recruitment, treatment confirmation, etc)

 Consultation fee: $200.00 per hour. 

Consultation and coordination of services may be a required and/or necessary part of your care. These services include time spent in phone consultation for more than 15 minutes with other professionals or agencies ( after you sign an authorization to release and exchange information) or attendance at school conferences, such as IEP meetings or department of social services. There may be times that you want me to read documents that will help to understand your needs, currents status, and/or to plan your individualized treatment; If reading such documents requires extensive time a fee will be charged per hour of consultation.

 Legal proceeding fees

You will be expected to pay for all professional time spent in preparing legal documents on your behalf, consultation with attorneys or other professionals, or attending court. Payment for such services will be required on the date of the service or in advance as needed. Services that are not for medical necessity will not be reimbursable by insurance plans.

Subpoena: $300 for each subpoena that is issued to Niosoty M Baptista de Paparella MA, LPA – regardless of the issuing source of the subpoena and regardless of the status of treatment.

Communications in regards to a legal matter: $250 per hour or 15 minutes portions thereof for communication in regards to a legal matter.

Reports: $250 per hour or 15 minutes portion for time required to prepare a report requested or required for legal matters.

Copying: Expenses for copying records or documents requested will be charged at $0.75 per page for the first 25 pages, $0.50 per page for pages 26 to 100, and  $0.25 for each page in excess of 100 pages plus $75 per hour for administrative fee. Expenses for mailing documents will be charged at the current rate charged by the postal services.

Electronic transmission: Records will be released by fax only. A standard fee of $75 for administrative fee will be charged.

Payment for release of records in any format must be prepaid by the patient or the patient’s designated representative.

Testimony: While I typically do not make court appearances, if I have to present testimony in court then time spent in preparation for, on-call, in travel, while waiting, and in attendance at hearings, trials, or depositions, is billed $450 per hour. A minimum of four hours must be reserved and paid two weeks in advance for appearance at deposition, hearing, or trial. If cancelled (release from subpoena) with at least five business days’ notice, 100% is refunded; if cancelled with less than five business days but more than two business days, 50% refund of the advance payment will be made; no refund will be made if cancelled with less than two business days’ notice. Failure to comply with the policy may result in a motion to quash or other actions.

Mental Health Evaluations: If a mental health evaluation is requested for legal matters, a separate contract must be signed instead of therapist and client consent for treatment.
